Maria is working on a medical surgical unit and receives a direct admission from the internal medicine clinic. She arrives at the patient’s room to complete the initial admission assessment.   1. Patient ambulates with assistance of wheeled walker.
  2. Patient’s BMI is outside of the normal range.
  3. Patient appears unkempt and has strong body odor.
  4. Patient is experiencing increased difficulty breathing.
