Antepartum Care Case Study A[1]

A pregnant client enters the clinic for her 38-week routine OB visit. The client is measuring 35 centimeters from her symphysis pubis to the top of her fundus. The client reports having increased vaginal discharge. The client states feeling happy that she can breathe a little easier now. However, she states she urinates much more frequently today. She has had a weight gain of 2 lbs in the past two weeks.

Vital Signs: BP 140/94, T 37 centigrade, pulse 78, resp 18 and fetal heart tones are 156.

Hgb: 10.1 mg/dl

Blood type: A negative

Urine dipstick test: Trace of protein and 1+ glucose

Group B strep status: Positive

Rubella status: Non-immune.

Hepatitis B &C status: Negative.

  1. Identify abnormal findings requiring follow-up.
  2. What health teaching and anticipatory guidance should the nurse offer if the client reports the following symptoms?

Antepartum Case Study B[2]

A multipara is in for a clinic appointment at 32 weeks and appears apathetic. She is worried how her two year old will accept the new baby and also expresses concerns regarding her partner’s involvement in the pregnancy as he has been distant to her.

  1. How does mutiparity affect the maternal tasks of pregnancy?
  2. How should the nurse respond therapeutically to the client’s concerns?

Antepartum Case Study C[3]

A woman gave birth three years ago to twin girls at 38 weeks gestation. She had a spontaneous abortion last year at 12 weeks’ gestation. She thinks she may be pregnant now because she has missed a menstrual period and has nausea. The pregnancy is confirmed with a positive urine pregnancy test. Her LMP was August 22nd.

1. What is her gravida para history?

2. Explain a non-stress test and how to interpret the findings.

3. What is a reactive and non-reactive non-stress test?

4. Explain the biophysical profile.
