13.5 Spotlight Application

The following spotlight application provides an example applying the nursing process to assessing and providing care to a school-aged child at a well-child check.

1. What assessment data should the nurse and provider collect?

Weight Sleeping and sleep hygiene
Body mass index (BMI) Screen time
Scoliosis screening Start of menstruation
Hearing screening
Signs of breast development or pubic hair

2. Based on the assessment data, what child-related nursing diagnosis applies?

Readiness for enhanced health self-management

3. What is an appropriate goal for this child?

Client will practice healthy behaviors of exercise, nutrition, social activity, sleep, immunizations, dental care, and safety appropriate to their age.

4. What nursing interventions could the nurse implement?

Complete screenings appropriate to age

Measure vital signs, height, and weight

Administer vaccines according to the CDC schedule for client’s age and prior vaccines

Provide anticipatory guidance on the following:

Exercise

Nutrition

Social activity

Sleep

Safety equipment like helmets and seatbelts

Access to guns

Social media

Dental care

5. How will the nurse evaluate if the interventions were effective?

Client’s BMI is within the normal range.

Client reports 60 minutes of physical activity per day.

Client reports eating a well-balanced diet.

Client reports interactions with friends on a regular and healthy basis.

Client reports sleeping 9-12 hours per night.

Client and parents report safety rules for the child and family.

Client reports they have a cellphone but not a smartphone. They do not use social media.

Client reports a yearly teeth cleaning and dental check-up.

6. If the child is appropriately progressing toward the goal, what should the nurse do next?

Document that the child is progressing toward the overall goal and if the outcomes were met, partially met, or not met.

The provider will set a time to reevaluate progress at a well check one year later.
